Note: A person's risk of coronary heart disease is affected by many factors. They include cholesterol levels, family history, weight, exercise tolerance, presence of other diseases such as diabetes, smoking, and several other factors. If you have any questions or concerns about your risk of heart disease, please consult with your personal physician.

Please enter your unit number and date of birth below. You must enter a 4 digit year in order to successfully retrieve your results. For example: If your birthday is May 6, 1951, you must enter 05/06/1951 into the date of birth box to successfully retrieve your results.
